Congenital curved penis: why does it occur and is it necessary to operate?

Congenital curved penis is a consequence of a genetic condition. Men are born with the curvature and usually notice it during puberty, when penile development occurs and erections become more frequent.
Peyronie’s disease, trauma and fractures are different because they are acquired throughout life and can be accompanied by nodules, in addition to causing discomfort, pain and making it difficult to maintain an erection.
It is common for diagnoses for congenital curved penis to be postponed due to the embarrassment of young people in discussing the issue with their parents.
Why congenital curved penis occurs
For the erection and rigidity of the penis to occur, elasticity and compliance of the tissues are needed. It expands in all dimensions when filled with blood. In a penis where there is no congenital curvature, the tissues are symmetrically elastic and the erection is “straight” (or up to 30º).
In congenital cases, there is a congenital asymmetry of the corpora cavernosa, with increased compliance of the tissues on only one side, that is, when one side is larger and more elastic than the other. Tortuosity occurs up, down, or to one side.
Is it necessary to operate on the congenital curved penis?
Not necessarily. When the angle of curvature is not so high, it is not necessary to look for surgical solutions. However, there are problems associated with a more pronounced curvature, such as:
- Difficulty, pain or impossibility of penetration;
- Pain and discomfort for the partner;
- Restriction to few sexual positions and movements;
- Fibrosis formation on the opposite side to the curvature;
- Premature ejaculation as a result of sensitivity in the glans;
- Psychogenic dysfunction, caused by fear, shame and loss of self-confidence.
The corrections of the congenital curved penis
Techniques vary according to the shape, type and degree of curvature. The main ones used are:
- STAGE
This technique makes small incisions in the limb, followed by mini sutures that correct the curvature without affecting the size of the limb during and after the procedure. The size is little affected if we consider the size of the erect penis measured from the pubis to the tip of the glans before the procedure. Some cases may have small losses, such as 0.4cm. There is no risk of erectile dysfunction and it can also be used in cases of stable Peyronie with curvatures in only one plane.
- Nebit
Here we have the classic treatment of these cases, but little used when the surgeon knows the Stage technique. With Nesbit, there may be an opening of the corpora cavernosa and the presence of sutures is more conspicuous, remaining more apparent than is generally desired. Size reduction can also be observed more frequently. Every 30o of curvature is estimated to lose or adjust the longer side by about 1.6cm.
- Grafting
Aimed at cases of more pronounced curvature and when the patient is very concerned about size. This procedure manipulates the side of the curvature with the placement of a graft to make the penis finally straight. It is an aggressive and more complex procedure. No patient at Instituto Peyronie was referred for this procedure given the possibility of erectile dysfunction resulting from the surgery, in addition to other complications.
